We may also collect information when you contact us through our website contact form, by telephone, email or social media to respond to your enquiry and determine whether physiotherapy is appropriate for you.
We store your personal information securely and only authorised members of Accelerate Physiotherapy, together with trusted third-party service providers acting on our behalf where necessary, have access to it.
Use of your information
We hold and process the personal information you provide in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. We use your personal information to provide physiotherapy assessment, treatment and ongoing care, manage appointments, communicate with you regarding your care, maintain accurate clinical records, process payments where applicable, and meet our legal and professional obligations.
We process your personal information because it is necessary to provide the healthcare services you have requested and to fulfil our contractual obligations to you. As part of providing healthcare, we also process special category data relating to your health. This is permitted under UK GDPR where processing is necessary for the purposes of medical diagnosis, the provision of healthcare and treatment, and the management of healthcare services by registered healthcare professionals who are subject to a duty of confidentiality.
We will only use your personal information for the purposes for which it was collected unless we are legally required or permitted to do otherwise.
We will not use your personal information for marketing purposes without your consent.
Sharing information
We will only share your personal information where it is necessary for your care, where you have given your consent, or where we are required or permitted to do so by law. This may include your GP, other healthcare professionals or your insurer where appropriate.
We will never sell your personal information to third parties.

We use trusted third-party service providers to support the delivery of our services. These include:

• Gmail and Proton Mail – email communication.

These providers process personal information where necessary to provide their services and have their own privacy policies. We only share the minimum information necessary for them to provide those services.

We take appropriate technical and organisational measures to protect your personal information against unauthorised access, loss or misuse. Whilst we take reasonable steps to protect information transmitted by telephone, email or the internet, no method of electronic transmission can be guaranteed to be completely secure.
If you contact us through social media platforms such as Facebook or Instagram, please be aware that these services operate under their own privacy policies.
Your individual rights
Under the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, you have the following rights regarding your personal information:
Right to be informed
You have the right to understand what personal information we collect, why we collect it, how we use it, who we may share it with and how long we keep it.
Right of access
You have the right to access the personal information that we hold about you, to know what we hold, in what format it is in and to have a copy of it if you wish. This is called a subject access request and it is free of charge. For more information on a subject access request, please email us acceleratephysiotherapy1@gmail.com
Right to rectification
You have the right to request that we correct any inaccurate or incomplete personal information we hold about you. We may ask for evidence to support your request where appropriate. Please note that clinical opinions recorded at the time of treatment may not be altered if they accurately reflect the assessment made, even if a diagnosis changes following further investigation.
Right to erasure
You have the right to request that we delete your personal information in certain circumstances. However, this right is not absolute. As a healthcare provider, we are often legally and professionally required to retain clinical records and may be unable to erase them where there is a legal obligation or another lawful basis for continuing to process your information.
Right to restrict processing
You have the right to request that we restrict the processing of your personal information in certain circumstances. Where applicable, we will inform you of the outcome of your request and any restrictions that apply.
Right to object
You can object to the processing of personal data in certain circumstances. You have the absolute right to object to the processing of personal data if it is for direct marketing purposes.
Right to data portability
You have the right to request a copy of the personal information you have provided to us in a structured, commonly used and machine-readable format where this right applies. You may also request that this information is transferred to another organisation where it is technically feasible.
Handling requests for individual rights
We aim to respond to requests relating to your data protection rights within one month, although this may be extended where permitted by law for complex requests.
To exercise any of your rights, please contact us at: acceleratephysiotherapy1@gmail.com
How long do we keep your personal information?
We only retain your personal information for as long as it is necessary to provide healthcare services and to comply with our legal, regulatory and professional obligations. Clinical records are normally retained for a minimum of 8 years following your last appointment, in accordance with current NHS Records Management Code of Practice guidance. Records relating to children are retained until the patient's 25th birthday (or 26th birthday if treatment ended when they were 17), or for longer where required by law or where there is an ongoing legal claim.
Once the appropriate retention period has expired, your records will be securely destroyed or permanently deleted so that they cannot be reconstructed or accessed.
Data Protection Compliance
Antony Chan and Leonie Shipley are responsible for data protection compliance within Accelerate Physiotherapy Ltd.

If you are unhappy with how we have handled your personal information, please contact us so that we can try to resolve your concerns. You also have the right to lodge a complaint with the Information Commissioner's Office (ICO). Further information can be found on the ICO website at www.ico.org.uk.
